Patient communication consent

Hospital and clinic staff: read the following to the patient before recording verbal permission. Record the patient’s answer separately for text and email. Unchecked consent means no permission.

May STRAC SERS send you appointment coordination texts about your first meeting, including requests, confirmations, changes and reminders? Message frequency varies; message and data rates may apply. You can reply STOP at any time to unsubscribe or HELP for assistance. You may also call the texting number and press 1 for CHCS or 2 for Be Well. The text inbox is not monitored. Your permission is optional and does not affect your care. The privacy policy and SMS terms are available at sers.strac.org/privacy and sers.strac.org/sms-terms.

For email, ask separately: “May STRAC SERS email you appointment coordination information? This is optional and you can ask your clinic to stop emails at any time.”
