SERS is operated by the Southwest Texas Regional Advisory Council (STRAC) to coordinate referrals and first meetings with CHCS and Be Well.

Information used for care coordination

Authorized hospital staff submit patient identity, contact details, eligibility, referral context and documents. Authorized program staff use this information to contact patients, coordinate appointments and maintain treatment and follow-up records.

Optional communications

Text and email permission is optional and is not a condition of receiving services. Mobile numbers, SMS consent and opt-in information are not shared with third parties or affiliates for marketing or promotional purposes. Service providers may process information only to deliver and operate this program’s communications.

Your choices

Reply STOP to unsubscribe from text messages. Reply HELP for assistance, or call the SERS text number and choose your clinic. Contact your clinic to update your information or email preferences. Do not send sensitive medical information by text.
